Integrated Primary Care

Initial intake is a 60 minute intake appointment. This visit allows the provider to listen to the story, ask questions and understand you as a whole person. Medications, past medical history, surgical history, family history, lifestyle, sleep, nutrition, social history, mental health history are discussed. Comprehensive labs are ordered at this visit and included in the visit cost. Target problems and goals for the next visit are discussed. The follow-up visit where labs are interpreted and recommendations are made is scheduled during this appointment and is included in the initial visit cost. Prescription refills are available at this appointment on an individual basis based on your current relationship with your primary provider, mutual preferences and provider recommendations.

Initial Visit:  $450 (includes comprehensive labs and first follow-up visit).

Follow-up Visit:  $250 (45 minute appointments)
